Medical Director - Coordinated Care (Family Medicine)

Position Highlights: 

• Work-Life Balance: Flexible schedule with a 3-5 day work week and just one day per week in a structured correctional care setting. No call, no holidays, or weekend requirements.
• Desirable Practice Setting: Modern, patient-centered clinic with integrated care delivery
• Ideal Work Environment: 10–12 patients per day supported by experienced APP staff
• Leadership Opportunity: Lead and grow a large health system’s coordinated care program with meaningful impact on patient outcomes

Compensation & Benefits:

• Competitive Compensation: Strong base salary depending on experience
• Generous Quality Bonus: Incentives tied to quality outcomes and program success
• Sign-On Bonus: $100,000
• Relocation Stipend: Up to $15,000 based on location
• Student Loan Repayment Assistance: System-funded and paid directly to lender

Qualifications: 

• Degree:  MD/DO
• Certifications:  Board certification in Family Medicine, Addiction Medicine, Internal Medicine, or IM/PEDs
• Licensure: MN license or ability to obtain
• Skillset: Interest in or comfort Addiction Medicine and underserved populations
• Experience: Open to residents and fellows, leadership interest encouraged
