Our team is dedicated to providing exceptional rehabilitation services to our patients. We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehabilitation team. Whether you’re treating post-surgical patients, those recovering from injuries, or those managing chronic conditions, you’ll play a key role in improving mobility and quality of life. • Completes screening process of all patients.
• Evaluates and treats all patients who could benefit from intervention as referred by an approved referral source.
• Appropriately document progress toward identified physical therapy objectives.
• Communicate patient status and needs to the patient, patient’s family, relevant staff members and other professionals included in the care plan of the patient.
• Actively integrates therapist’s role into patient care team.
• Provides education, guidance and support of other health care professionals and the general community about physical therapy.
• Make appropriate referrals to other members of the rehabilitative team.
• Develop and implement new programs/techniques to enhance physical therapy services and facility utilization.
• Adheres to established facility policies and procedures related to environmental safety, orderliness, risk management, infection control and emergency situations.
• Maintain awareness of issues related to the profession of physical therapy and the healthcare environment.
• Perform other functions as required to fulfill the expectations of the Physical Therapist role.

Requirements

• Minimum Required Education/Experience: Master’s degree in Physical Therapy accredited by the American Physical Therapy Association (APTA).
• Note: An acceptable combination of education, training and experience that provides the above knowledge, abilities and skills may be substituted.
• Licenses or Certificates: If driving, a valid driver’s license and ability to be insured as a facility driver. Valid Colorado Physical Therapist license or eligible to obtain.
