·       100% outpatient with minimal call by phone only

·       Flexible schedule with 36 patient contact hours per week

·       $350,000 plus potential, $80,000 sign-on bonus, 35 days PTO

Highlights:

• 1.0 FTE- 36 patient contact hours per week (flexible with schedule)
• Team consists of 2 physicians, 2 Nurse Practitioners, RN's & MA's
• 100% outpatient- patients referred to outpatient for infusion (not done within the Rheumatology department)
• Call: Every 3rd week (minimal and by phone call only) 
• Outreach Clinic at Critical Access Hospitals (won’t be right away- need to support our 2 main campuses in at TCN in Neenah and at Encircle in Appleton)
• EPIC EMR used throughout health system

Offer Details:

• Base Salary: Year 1- $275,000 + production, Year 2- $275,000 + production +valued based incentives. Year 3+ Floor $263,203 + production + value based incentives
• Sign On Bonus: $80,000 forgiven over 4 years
• PTO: 35 days inclusive of vacation, CME and holidays
• CME: $4,500
